Possible ingrown hair on paw

Our 9 year old CKC been to vet with sore on top of paw. He gets in such a state at vets they have not really sorted it. He?s got antibiotics and pain killers . We have pulled some hair out of it and it looks like there?s more I n there, we can?t seem to pull it out. We are soaking in Epsom salts, poulicing it and putting magnesium sulphate on it. He can?t be put under anasthetic because of his heart. Any suggestions. It looks really sore now and he gets himself in a right tizzy when we try to pull it out
